WE ARE SORRY TO ADVISE THAT WE ARE UNABLE TO PROVIDE OUR USUAL ‘OPEN SURGERY’ SESSION ON MONDAY 20th MAY. PATIENTS WHO BELIEVE THAT THEY REQUIRE AN URGENT APPOINTMENT ON THIS DAY SHOULD CALL THE PRACTICE BETWEEN 08:00 and 08:30 IN ORDER THAT THEY CAN BE ASSESSED BY THE TRIAGE NURSE AND AN APPROPRIATE RESPONSE PROVIDED.

NEW PARTNER - DR HASAN HABIB 

WE ARE DELIGHTED TO FORMALLY ANNOUNCE THE APPOINTMENT OF DR HASAN HABIB AS A NEW PARTNER AT THE PRACTICE. DR HABIB JOINS US FROM ANOTHER SWINDON PRACTICE ON 10th JUNE 2013 AND WILL, WE ARE SURE, PROVE A GREAT ASSET TO THE PHOENIX SURGERY.

THE LAST YEAR HAS BEEN A DIFFICULT ONE FOR THE PRACTICE WHILST WE HAVE BEEN OPERATING WITH A PARTNERSHIP VACANCY BUT WE ARE CONFIDENT THAT DR HABIB'S APPOINTMENT WILL BRING IMPROVED STABILITY AND APPOINTMENT AVAILABILITY.


This surgery website is used to present all our patients with up to date news and information about our practice. We will keep it current with any developments or other details that we feel are relevant to you.

The Phoenix Surgery in Swindon provides patient-centred traditional 'family doctor' care and is a founder member of the Family doctor Association. As a service to patients we also offer extended surgery hours on many Tuesday and Thursday evenings for pre-booked appointments. 

Check_throatThe practice is open for the registration of new patients living in West and North Swindon, as well as the nearby villages. You will need to call into the surgery to register with us and book a new patient health check.

As well as a source of information about our practice, the staff and the services we provide, we hope you will use this website as a useful resource for other health-related information - the Video Library, Long Term Conditions and Minor Illness sections can be accessed from the links on the right hand side of this page and have literally hundreds of interesting and informative videos available.


ONLINE APPOINTMENT BOOKING 

Following our recent computer system change, the previous Emis PatientAccess online booking facility has now been replaced by an improved service called SystmOnline. M ost exisiting account holders for Emis PatientAccess can still log into SystmOnline using their previous details as follows:

To do this you will need to add _1694 onto your EMIS Access ID to complete your new Username.  e.g. if your ACCESS ID is 12345, and your PRACTICE ID is 1694, your new Username becomes 12345_1694. (Please note, there is an underscore “_” between the two IDs). Your password will remain the same.

Out of Hours
On weekdays between the hours of 6.30pm and 8.00am, and all day and night at the weekends and on bank holidays, services are commissioned by the Swindon Primary Care Trust.  Please call 01793 646466.  After assessment of your symptoms you may be invited to attend the Out of Hours Centre which is located in the Clover Unit at The Great Western Hospital. 

Home Visits
If possible please try to telephone reception before 10am if you require a home visit. A doctor or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.

Cancelling Appointments
If you are unable to attend an appointment with one of the doctors or nurses, please telephone as soon as possible.
